Official Rules of the Name Creative Gifts Of Healing's Newsletter Contest

Contest open to legal residents of the United States who are 18 years of age
or older and shall be construed and evaluated according to U.S. Law. Do not enter
if you are not located in the United States. Void in Puerto Rico and where
prohibited by law.

• You do not need to buy anything to be eligible for this Contest. Just follow
  the Official Rules below for all the details.
  
• To enter, simply visit the CreativeGiftsofHealing.com's website and complete the
  online registration form with all required information. A original name must be submitted for your entry in order to be considered a valid entry. All entries
  without a valid name will be disqualified. Entrants may submit as
  many names as desired, but each entry must be submitted separately. Each
  name may only be submitted once.
  
• Judging and criteria for winning: All entries will be reviewed and evaluated by a
  panel of judges at Creative Gifts of Healing. Names will be judged on the
  following weighted basis: Originality - 30%, Creativity - 40%, and Adherence to the Theme– 30%. All names must be original, previously unpublished and
  created by the entrant her/himself. Ties will be broken on the basis of who
  entered first. Judges decisions will be final and binding.
  
• This Contest is sponsored by CreativeGiftsOfHealing.com. By entering this
  Contests, participants agree to release, discharge and hold harmless
  CreativeGiftsOfHealing.com its officers, directors, employees, agents, legal
  representatives, affiliates, subsidiaries, retailers and agencies and their
  respective officers, directors, employees and agents, from any damages which
  may arise out of participation in this Contest, or out of the acceptance,
  use, misuse or possession of prizes. By participating, you agree to these Official
  Contest Rules, and to the decisions of the Sponsor which are final and
  binding in all respects. Void where prohibited by law, rule or regulation. This
  Contest is open only to legal residents of the United States who are at
  least 18 years of age, with the exception that this Contest is void in
  Puerto Rico and all territories and protectorates of the United States, and where
  specifically prohibited by law.
• The Prize is one (1) copy of Caroline Myss' Journal of Inner Dialogue book, ARV $29.95. From among all entries received during the contest period, one winner
  will be chosen. Sponsor also reserves the right to substitute any Prize for its
  Approximate Retail Value (ARV) with a like Prize. No cash redemption.
• If you are selected as the winner and are notified by
  CreativeGiftsofHealing.com, you must respond within 14 days, identity must
  identically match the information in CreativeGiftsofHealing.com's database for a
  prize to be awarded. Entries will be deemed made by the person under whose
  name the entry was registered, regardless of who actually submitted the entry.
  Noncompliance within this time period may result in disqualification. Winners
  may be required to provide picture proof of identification, which may include a
  driver's license, passport, a voting card or similar government issued
  identification. Return of any prize/ prize notification as undeliverable may result
  in disqualification.
  
• Contest ends on December 15, 2004 at midnight. Winner will be selected from among all qualified entries received. Judging will be begin on or about December
  18, 2004. Winner will be notified on or about December 18, 2004 by email.
  
• All taxes and fees on prizes won are the sole responsibility of the winner.
  
• Acceptance of any prize constitutes permission for CreativeGiftsofHealing.com
  and its affiliates to use winner's name and likeness for advertising and
  promotional purposes without additional compensation unless prohibited by
  law.
  
• By entering, each participant agrees to release and hold harmless
  CreativeGiftsofHealing.com, its legal representatives, any prize providers, and
  all sponsors and each of their respective parents, subsidiaries, affiliates,
  directors, officers, employees, advisors, assignees, licensees, representatives,
  agents and any other person or entity associated with this Contest from
  any and all liability for any injuries, loss or damage of any kind arising from or in
  connection with the Contest or any prize won. Restrictions, conditions, and
  limitations apply.
  
• If for any reason the Contest program is not capable of running as
  planned, CreativeGiftsofHealing.com reserves the right to extend the entry
  date, cancel, and/or postpone the Contest at its sole discretion.
  CreativeGiftsofHealing.com reserves the right to change any or all the rules at
  its sole discretion.
  
• Sponsors are not responsible for lost, late, misdirected, incomplete, interrupted
  or unavailable network, server or other connections, miscommunications,
  computer or software malfunctions or telephone transmission problems or
  technical failures, garbled transmissions or typographical or other errors or
  malfunctions of any kind whether human, mechanical, electronic or otherwise.
  Sponsors, in their sole discretion, reserve the right to disqualify any person
  tampering with the entry process, the operation of the website or otherwise in
  violation of the rules. Sponsors further reserve the right in their sole discretion
  to cancel or suspend the Contest or any portion thereof should virus, bugs
  or other causes beyond control of Sponsor, corrupt the administration, security
  or proper play of the Contest.
• By submitting this entry, I agree to the following: I agree
  to allow my name and entry to be published on this Web site, newsletter
  and in other Creative Gifts of Healing materials. I certify that I am the sole
  author or sole owner of the material I am submitting to Creative Gifts of
  Healing and that such material is not based on or derived from any
  copyrighted, confidential or proprietary material of any third party. I agree
  to indemnify Creative Gifts of Healing for all damages and expenses that may
  be incurred in connection with the entry submitted. Creative Gifts of Healing
  and its licensees may reproduce, distribute, publish, display, edit, modify, create
  derivative works and otherwise use the materials for any purpose in any
  form and on any medium on a worldwide basis and without additional
  compensation. I agree to be bound by the decisions of the judges,
  which are final. Creative Gifts of Healing reserves the right to edit, modify,
  adapt, publish, use and reproduce any and all materials regardless of
  winning or not, without further compensation.
  
• Any attempt by any individual, whether or not a contestant, to damage,
  destroy, tamper with or vandalize this website, or otherwise interfere with the
  operation of this Contest, including but not limited to mechanical entry, is a
  violation of criminal and civil law and will be disqualified.
